Addys Diabetes Clinic is seeking a compassionate, organized, and results-driven Diabetes Care & Nutrition Program Manager to manage diabetes patients, design personalised meal plans, and serve as an accountability partner for clients enrolled in our Diabetes Reversal Program.
The ideal candidate will play a critical role in patient success, engagement, and long-term lifestyle change.
Key Responsibilities
- Manage and support diabetes patients within the Addys Diabetes Clinic.
- Create personalized and group diabetes-friendly meal plans aligned with our reversal and lifestyle management programs.
- Serve as an accountability partner and coach for participants in the Diabetes Reversal Program.
- Monitor patient progress, track compliance, and provide regular follow-ups and encouragement.
- Educate patients on nutrition, lifestyle habits, and blood sugar management strategies.
- Maintain accurate patient records and progress reports.
- Collaborate with the clinical, nutrition, and operations teams to ensure quality patient care.
- Support clinic visibility and engagement through social media content, patient education posts, and community interaction.
- Demonstrate leadership by guiding patients, supporting junior team members, and upholding clinic standards.
Requirements & Qualifications
- Background in Nutrition, Dietetics, Nursing, Public Health, or related health sciences.
- Experience working with diabetes patients or lifestyle disease management is a strong advantage.
- Strong knowledge of diabetes nutrition and blood sugar management.
- Excellent communication, interpersonal, and coaching skills.
- Demonstrated leadership and patient management abilities.
- Comfortable using social media platforms for education and patient engagement.
- Professional, smart, and able to work effectively in a corporate healthcare environment.
- Empathetic, organized, and detail-oriented.

Pay: ₦100,000.00 – ₦150,000.00 per month
